Little Falls Hospital, part of the Bassett Healthcare Network, recently announced that its Dolgeville and Newport primary care centers have received patient-centered medical home recognition.
The PCMH is a care delivery model whereby patient treatment is coordinated through their primary care physician to ensure they receive necessary care when and where they need it in a manner they can understand.
The objective is to have a centralized setting that facilitates partnerships between individual patients and their personal physicians, and when appropriate, the patient’s family.
Registries, information technology, health information exchange and other means facilitate care to assure patients get the care when and where they need it.
“This puts our primary care centers among a select few practices nationwide to be recognized for our ongoing commitment to advancing quality and patient-centered health care,” says Heidi Camardello, vice president of patient care services and chief nursing officer.
Dolgeville Primary Care Center is located at 9 Gibson Ave., Dolgeville. Newport Primary Care Center is located at 3085 Bridge St., Newport.
